The Medical Specialist Group and secondary healthcare in the Bailiwick

The Medical Specialist Group provides emergency and elective specialist medical services for the Bailiwick of Guernsey, in partnership with the States of Guernsey’s Committee for Health & Social Care.

The MSG’s story

Up until the 1990s, medical care in Guernsey was provided privately by GPs on the island, some of whom had specialist interests and provided hospital care for their practice’s patients in the publicly-owned hospital.

With the medical advances of the 1980s and as secondary care became more specialised, this model became untenable. So, in 1992 a group of 19 doctors based in the island’s GP practices joined together to provide emergency and elective specialist medical services to islanders as the Medical Specialist Group. 

A few years later, in 1996, the MSG agreed a contract with the States of Guernsey to provide specialist healthcare free at the point of delivery to everyone in Guernsey, Herm and Alderney. Only emergency department care and general practice remain private. 

The Secondary Healthcare Contract

There are two main ways through which people can become a patient with us: Becoming a patient - MSG.

  • As a contract patient. If an islander lives in Guernsey, Alderney or Herm, they are entitled to receive secondary healthcare free at the point of delivery under our contract with the States of Guernsey.
  • As a private patient. Islanders can choose to be seen as a private patient for a service we offer under the States contract and receive extra benefits. Or they can choose to be a private patient because they would like a treatment that is not covered by the States contract – what we call a non-contract service.
  • If someone doesn’t live in Guernsey, Alderney or Herm, we can see them as a private patient.


Key Performance Indicators

The MSG works in partnership with the Committee for Health & Social Care (HSC) through the secondary healthcare contract.

The current contract, which began in 2018, includes key performance indicators (KPIs) which assess the services provided in four key areas: professional compliance, productivity and quality, patient safety and experience and sustainable service.

The MSG Today

The MSG provides a consultant-only service which ensures the highest standards of care are available to islanders. 

Since 1992, its services have expanded and developed in response to local healthcare needs and the increased specialisation of medicine and surgery. There are now 55 consultants and a team of almost 100 support staff.

Owned and run by doctors since the beginning, the MSG is now a limited liability partnership (LLP), a structure commonly used by professional groups such as lawyers. Consultants usually join the MSG as associates and are later invited to make a long-term commitment to the island’s healthcare by joining the partnership.

Partners, who are self-employed, elect lead partners to serve on a Management Board which manages the group on behalf of the partners. It has the delegated authority to make certain decisions while other decisions go to the whole partnership for a vote.

Directorates

The MSG is organised into four directorates: Adult medicine, Anaesthetics, Surgical, and Women and Child Health.

The chair of each directorate sits on the Management Board along with the MSG chair, the clinical governance lead partner and the finance lead partner.

The MSG’s base is at Alexandra House and Mill House, where most outpatient clinics are held, but consultants spend much, if not most of their time at the nearby Princess Elizabeth Hospital.
